First Visit Checklist

Print this page or keep it on your phone. Tick things off as you go — nothing on it is sent to Rosecare.

Bring with you

  • A list of every medication and supplement you take, with doses (or the bottles themselves)
  • The names of other providers involved in your care
  • Any prior psychiatric records or a summary of past treatment, if you have them (not required)
  • Insurance card and photo ID, if you plan to use insurance
  • Reading glasses or hearing aids if you use them
  • A family member or caregiver, if you would like them involved

Think about beforehand

  • What made you decide to reach out now
  • When the difficulties started and what has changed
  • What has helped before, and what has not
  • What you hope will be different after treatment
  • Questions you want to ask — write them down

Practical

  • Confirm the visit format (in person or video) and the time
  • Plan to arrive a few minutes early, or test your video connection
  • Allow time afterward; first visits are unhurried
  • Know who to call if you need to reschedule

If it is a video visit

  • A private, quiet space
  • A charged phone, tablet or computer with a working camera and microphone
  • A reliable connection; have the office number handy in case the call drops

Not sure about something? Call the office during business hours. For an emergency, call 911 or call or text 988.
