Condition

Dementia-Related Behavioral Changes

Agitation, aggression, sleep disruption, paranoia, or distress in Alzheimer's disease, vascular dementia, or Lewy body dementia.

What it can look like

  • Agitation, pacing, or aggression that is new or escalating
  • Sundowning — confusion and distress that worsens late in the day
  • Sleep reversal or night-time wandering
  • Suspicion, accusations, or seeing things that are not there
  • Resistance to care that puts the person or caregivers at risk
  • A rapid change in behavior that may signal a medical problem

Experiencing some of these does not mean you have this condition. Many symptoms overlap, which is why a careful evaluation matters.

How it is evaluated

Behavioral symptoms in dementia have causes, and finding them is the first step. Pain, infection, constipation, dehydration, medication side effects, and changes in routine or environment all commonly drive behavior. Justina evaluates the behavior alongside the person's medical status and medication list, and involves caregivers in describing what happens, when, and what seems to help.

How treatment may help

The goal is calm and safety with the fewest medications possible. Non-drug strategies — adjusting routines, environment, and communication — come first, and caregivers are coached on de-escalation approaches that fit the person. Justina treats behavioral disturbances in Alzheimer's disease, vascular dementia, and Lewy body dementia.

The role of medication

Medication is used judiciously, at the lowest effective dose, and reviewed regularly for continued need. Some medications commonly used for behavior carry particular risks in older adults, and Justina discusses those risks openly with families before recommending them.

When to seek urgent help. If a person with dementia becomes suddenly and severely confused, aggressive to the point of danger, or is medically unwell, call 911 or seek emergency care — sudden change often signals an acute medical problem.
